Depending on nation and their set up there are options.
Sometimes unusual game tactics and strategies are in use.
I usually do 1v1 Ranked and totally ground up a helo-rusher
make sure 2 CV. 2 Manpads minimum...when you spot the rush beginning...because players will play it like total destruction. vape all CV to win, pump out cheap SPAAG and manpads.
If you are armored...and you have a force moving toward his base when the rush flies over...make sure at least 2 stabilized SPAAG and a Missile AA unit are with your force. You can also delay him because he either has to eliminate the threat or when he tried to engage a combination of missiles/SPAAG/and judicius use of tank AA [aka big gun trick] will knock enough helos out of his force and leave him vulnerable.
What surprises me though...is no Heli Rush with Mi-24D and transporting infantry. That would allow you to even in a worse case scenario drop off infantry to either delay or harrass so even if you lose your rush line...you have a really close second line behind that.

More rockets and an AGM that may actually hit, plus you can afford more mi8s.

True
45pt. unit. And the Malytuka is one of the odd ball high chance to hit helo ATGM at the moment. Fleyta/Flanga require you to push Mi-24/25 right on top of the target to get a hit. Between 2/3 of the missile range. But Rocket Pods have a rather nifty minimimum range. Which makes them useless if you are on the hunt for targets.
But then again Pol and NK can spam 20-30 units too. Haven't seen a rush with them yet. And there are the Mi-4 gunships too. But they are slower. Except the Little Birds of NK.
What has surprised me is that no player has completely taken advantage of the capacity of helos to fire ATGM/Missile Pods/Cannons/AA while landed. That might be the new flavor of the helo rush. And might make it really hard to counter.

The mi-4 speed is painful =( and lacks a way to damage armoured units. They are amazing to keep in reserve to stop any over-enthusiastic BLUFOR infantry though.
The Best Korea MD 500D helos really need some help, they're pretty redundant. Any combination of them is still far inferior to the Mi-8 Gunship or Mi-25. 90pts will get you all 3 MD's, which is a grenade launcher + 32 rockets + 4 ATGMS. The mi-25 can essentially do all that for 60 points.
90pts could also get you 2x Mi-8 Gunships instead... that is 192 rockets + 8 ATGMS... you also get the highest Strength for cost ratio with the Mi-8 and the NSVT gun actually has a higher suppression rating than the Yak-B. I am slightly obsessed with them if you can't tell already

How often does the helirush actually occur in ranked? And how many times has it actually worked?

According to this forum, a lot and a lot. With proofs, honestly. Just look at Helirusher posts.
